If you were recently trained in EBO, you need to complete at least one additional case study from the
User Guide.
When you are finished, fill out this checklist to confirm that you completed each step of training case study.
When the form is complete, click 'Submit.' Someone from the EBO Helpdesk will contact you after the checklist is submitted.
